Heart disease has been the number one cause of death in the United States since 1950.
It is also a condition heavily influenced by disparity, with some neighborhoods experiencing drastically higher rates of hypertension and cardiovascular disease. The Constellation Regional Collaborative aims to improve those numbers.
Constellation Quality Health recently launched the Constellation Regional Collaborative after receiving a five-year, CDC-funded cooperative agreement. The Collaborative aims to address disparities impacting hypertension prevalence across the Southeast – a key factor in rates of cardiovascular disease.
Our goal is to improve cardiovascular disease, hyperlipidemia, and stroke disparities in North Carolina and South Carolina by addressing these disparities and improving social determinants of health in communities with a 53% and higher prevalence of hypertension.

Our monthly ECHO series, launching in March 2024, will focus on evidence-based best practices, health equity, and community-clinical linkages to address improvements. Learn more.
Join and participate in the American Society of Hypertension (ASH), Hypertension Team Learning Collaborative
Powered by Constellation Quality Health, this regional learning collaborative will engage primary care participants in setting their outcome measures and goals. Each participant will receive a Certificate of Achievement to publicly display, signifying their commitment and achievement.
Help spread the word about our In-Clinic Pharmacist Program Expansion
This program expands existing community pharmacists’ involvement to include working alongside primary care providers to educate patients, complete medication therapy management, and screen for social determinant needs and additional chronic disease education opportunities.
Commit to working with our clients within the Constellation Community Hub (Hub)
The (Hub) is a community of care hub connecting community-clinical linkages for referred clients. This centralized bi-directional, closed-loop referral hub will allow providers to refer patients to meaningful evidence-based programs and community resources with a focus on reducing health disparities for prioritized populations and to provide equitable care delivery. The Southeastern United States has the unfortunate reputation of having the highest stroke and heart disease mortality rates in the nation. We anticipate a robust response from our Carolina providers and community in joining the Constellation Regional Collaborative to address inequalities in health care and reverse this trend.
